Appointment Information

New patient visits take anywhere from 45-60 minutes. We encourage you to show up 15 minutes prior to your scheduled appointment time. Our office is located off Polston Ave. in Post Falls directly across from the blue Kootenai Urgent Care building.

Please bring the following with you to your first appointment:

  1. Insurance card and photo ID

  2. Your completed intake form (if not submitted online)

  3. If available, please bring any lab or diagnostic reports from other health care professionals who have treated you for your current condition.

Insurance

We accept most major insurances including:

Don’t have insurance? Don’t worry. Optimal Chiropractic offers affordable cash rates to get you the help you need.

You are responsible for your insurance plan deductible and/or co-pay. If your insurance plan requires a co-pay, it is due on the date of service. If after billing your insurance company we discover you have not met your deductible, this additional amount will be billed to you.

We accept all major credit cards including Visa, Mastercard, Discover and Care Credit.

In the case of a personal injury claim, we will work with you to insure correct reimbursement from either auto insurance or workman’s compensation.
